How the low testosterone quiz works
How it works
This quiz is the ADAM questionnaire (Androgen Deficiency in the Aging Male), a 10-question screening tool published in 2000. You answer yes or no to each question about symptoms such as libido, energy, mood and sleepiness. It is designed to flag symptoms that are associated with low testosterone, not to measure your testosterone level.
How to read your result
The questionnaire counts as a positive screen if you answer yes to question 1 (decreased libido) or question 7 (less strong erections), or if you answer yes to three or more of the other eight questions. A positive screen means your symptoms are consistent with low testosterone and may be worth discussing with a provider. A negative screen means fewer of those symptoms were reported, but it does not rule low testosterone out.
Many of these symptoms have other common causes, including poor sleep, sleep apnea, stress, depression, thyroid problems, diabetes, excess weight and some medications. That is why symptoms alone are not enough to diagnose low testosterone.
When to talk to a provider
Low testosterone is diagnosed with symptoms plus blood tests. Levels change through the day and from one day to the next, so providers generally want more than one measurement before making a decision. If your result is positive, or if you have symptoms that bother you either way, a provider can review your history, order labs and discuss whether treatment is appropriate.
Prolong Health offers online men's health programs: testosterone therapy (adults 25 and older, in certain states) and enclomiphene (adults 18 and older, all 50 states and D.C.). Both begin with lab work and a provider visit, and a provider decides whether treatment is appropriate.
This quiz is a symptom screening tool, not a diagnostic test, and it does not determine eligibility for any treatment. Your answers are scored in your browser and are not sent to or stored by Prolong Health.

This questionnaire is a symptom screening tool, not a diagnostic test. A positive result means your symptoms are consistent with low testosterone and may be worth discussing with a provider — it does not confirm a diagnosis, which requires clinical evaluation and typically blood testing. This tool does not determine eligibility for any treatment; that determination is made by a licensed provider after a real medical evaluation.
